    This white paper is an assessment of best practices to support youth who age out of foster care.


    Foster youth who transition out of care at age 18 without reunification, adoption, or legal guardianship face a myriad of challenges and risks for poor outcomes. Supporting transitional-age foster youth will require a thoughtful and thorough assessment of the system that serves them, followed by strategic actions to ensure that they have the best possible start to life as adults in our community.
